Specialist Dyslexia Assessment: Full Intake, Terms & Consent Agreement

Diagnostic Assessment Terms, Conditions & Legal Consents
1. Structure of Fees and Service
Assessment Fee: The flat fee for a standard Dyslexia Diagnostic Assessment is £400. This covers the pre-assessment consultation, background questionnaire analysis, the standardized testing session, and the production of a SASC-compliant written report.
Scope of Practice: This evaluation determines whether an individual has dyslexia. It does not formally diagnose co-occurring conditions like dyscalculia, dyspraxia, ADHD, or ASD. If co-occurring neurodiverse traits are noted during testing, an onward referral to a GP or relevant specialist will be recommended.
Payment Terms: * A 50% non-refundable deposit (£200) is required at the time of booking to secure your assessment date.
The remaining 50% balance (£200) is due after the testing session, but strictly before the final report is released.
Reports will absolutely not be released or shared until full payment has cleared BACS.
2. Cancellations, Rescheduling, & Technical Requirements
Cancellations: Because your deposit is non-refundable, it will be forfeited if you cancel the assessment entirely.
Rescheduling (One-Time Policy): If you need to change your date due to illness or unforeseen circumstances, you must notify the assessor via email at least 24 hours in advance.
You are permitted one (1) opportunity only to transfer your non-refundable deposit to a rescheduled date (which must be booked within 3 months of the original date).
If you request to reschedule a second time, or cancel the rescheduled session, your deposit will be entirely forfeited and you will need to pay a new deposit to secure a future slot.
Remote Assessment Requirements (Online Sessions): For assessments conducted online via Zoom or Teams, the client must ensure:
Access to a laptop or desktop computer with a functioning camera, microphone, and stable internet connection. (Assessments cannot be run on mobile phones).
The individual is in a quiet, private room completely free from third-party distractions, interruptions, or background noise.
Absolutely no recording or screenshots of the testing process or confidential testing materials are permitted.
Assessor Rights: The assessor reserves the right to terminate or pause an assessment if the individual becomes excessively distressed, uncooperative, or if technical/environmental issues compromise the validity of the standardized tests.
3. Pre-Assessment Prerequisites
Mandatory Eyesight Check: The individual being assessed must have had a standard eye test conducted within the past 12 months. If they use glasses or contact lenses for reading, these must be present for the assessment.
Paperwork Deadline: All background questionnaires (including school inputs if applicable) must be fully completed and returned at least 1 week (7 days) before the scheduled assessment date. The assessor reserves the right to postpone the appointment if forms are missing.
4. Report Delivery & Service Conclusion
Turnaround Time: Your comprehensive, password-protected PDF report will be completed and sent to you within 4 weeks (28 days) of the assessment date.
Post-Report Email Support: We understand that digesting a diagnostic report can take time. To support you, you are welcome to submit brief, clarifying questions via email within 30 days of receiving your report. This complimentary service is designed to clarify specific terminology, scores, or recommendations.
Service Conclusion & Chargeable Consultations: Once the report has been emailed and the 30-day support window has passed, the assessment service is officially concluded.
Minor factual corrections to background history may be requested in writing within 30 days at no extra charge.
Any requests for extensive post-report consultations, phone/virtual meetings, or liaison with school staff will be treated as additional services and billed at an hourly rate of £50 per hour (or part thereof).
5. Parental Consent & Joint Responsibility (Professional & SASC Standards)
To protect all parties and comply with UK family law and professional SASC standards, this practice implements the following policies on Parental Responsibility:
Legal Responsibility Verification: The person signing this form must hold legal Parental Responsibility (as defined by the Children Act 1989) for the child being assessed.
Separated Families & Joint Parental Responsibility: In accordance with established professional guidelines and SASC standards, if both parents hold joint parental responsibility (irrespective of who is paying the fee), both parents must be aware of and consent to this diagnostic investigation.
If one parent with parental responsibility objects, the assessment cannot legally go ahead.
It is the responsibility of the commissioning parent (who books the assessment) to liaise with the other parent. By signing below, you declare that all individuals with parental responsibility have been notified and have agreed to this assessment.
School Questionnaire & Liaison Policy: In line with best practices, gathering educational feedback is a vital element of a diagnostic profile.
Rather than contacting the educational setting directly, the assessor will provide a dedicated School Questionnaire to you (the parent/guardian).
You are responsible for distributing this form to your child's class teacher or Special Educational Needs Coordinator (SENCo), coordinating its completion, and returning it to the assessor before the deadline. Direct contact with the school will not be initiated by the assessor.
Teenage Assent (Ages 13+): If the child is aged 13 or over, they have the legal right to give or withhold consent. You confirm that you have discussed this assessment with your child and that they are fully aware of and consent to participating.
6. GDPR Privacy & Data Retention Notice

Data Collected: Personal identity details, educational histories, and medical details (Special Category Data) are collected strictly to execute this diagnostic contract safely and effectively.
Data Retention:
Raw Material: All original paper test sheets, working notes, questionnaires, and temporary scoring audio/video recordings are permanently deleted or securely shredded immediately after the final report is compiled.
Final Report: Your completed final report is securely archived for 7 years (or until the child’s 25th birthday if they are a minor) to provide replacement copies if requested. After this time, it is permanently destroyed.
Sharing: Data is completely confidential and will not be disclosed to schools, colleges, or third parties without explicit, separate written consent.
